It is easy to begin the process of having a living trust. Merely complete the questionnaire and send it to us by e-mail, fax, or regular mail. When we receive it we will contact you.
For the section of the questionnaire dealing with assets, list the approximate amounts. You need not furnish names of institutions or account numbers.
We may eventually ask to see your current will or trust, if you have either, and copies of real estate deeds and property tax bills for real estate owned.
If your computer has Microsoft Word, you may download the questionnaire by clicking on the link or icon below. Choose "Open." When the questionnaire appears on your screen, you may type in the requested information, save it to your computer, attach it in an email message, and email it directly to me at david@foley-law.com
Or you can print it out and once it is completed, mail or fax it to me.
You may also mail or fax me your copies of grant deeds, property tax bills, and your will, if applicable.
